Jersey Shore Summer Recreation Program

What information you need for registration:
1. Medical insurance information
2. List of medical needs, such as allergies and dietary restrictions. (Jersey Shore Summer Recreation is not permitted to administer medications to children in the program)
3. Proof of residency
4. Names of people, with addresses and phone numbers, that will be permitted to pick your child up.
5. Proof of completion of kindergarten, if your child is 5.
